Faculty Test Request Form

Test proctoring requests should be made at least one week prior to the date that the test will be administered. Adherence to this deadline will ensure that the Testing Center will have sufficient time to locate a proctor and to confirm arrangements with both the student and the instructor.

The Missouri S&T Testing Center is a National Test Center and test materials are locked in a secured storage area. Any questions or concerns should be directed to Cheryl Downey-Eber at 341-4222, cherylde@mst.edu, 202 Norwood Hall, Fax 341-6179.

This form is to be completed and submitted by the instructor before the test deadline. (Seven working days' notice.)

While testing if evidence is found or an observation has been made in regards to cheating, testing will be immediately stopped and all materials confiscated.  Testing staff will notify the instructor at the time of the incident.  Thereafter, decisions regarding further action, associated with cheating, will be deferred to the instructor.

Instructors are responsible for delivering the tests to the Testing Center 202 Norwood Hall.
